About the Role
Our Behavioral Health Care Managers (BHCM’s) are at the core of what we do. Delivering care through the Collaborative Care Model, they improve the health of patients and serve as the project manager of the collaborative care team, which includes the patient’s oncologist as well as a consulting psychiatrist. By providing outcome-focused, brief behavioral health interventions that seamlessly integrate with the patient’s journey, our BHCMs ensure that mental health barriers are effectively addressed to achieve holistic physical health outcomes and enhance overall quality of life.
This is a fully remote opportunity supporting one or more of our partner practices, working Central or East Coast hours, 9:00am - 5:30pm CT / ET.
Primary Responsibilities
- Develop a trusted, effective working relationship with the partner oncology practice team
- Work within a partner oncology group to support patients, clinicians, and practice staff with the identification and intake of patients needing behavioral health support
- Support the mental health of cancer patients on an assigned patient caseload
- Provide brief behavioral interventions tailored for the cancer setting
- Screen and assess patients for common mental health and substance abuse disorders
- Facilitate patient engagement and follow-up care
- Provide patient education about common mental health and substance abuse disorders and the available treatment options
- Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
Requirements
- You hold one of the following licenses: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW/LSW), Clinical Professional Counselor (CPC), Licensed Professional Counselor (LPC) Licensed Psychologist (PH.D or PsyD), or Certified Oncology Social Worker (OSW-C)
- *We assist with expanding state licensures as-needed
- Prior experience interacting with physicians, collaborating with a multidisciplinary team
- Have a passion for delivering quality behavioral health care with a focus on 1:1 therapy
- Proven ability to operate with a high degree of autonomy, virtual remote environment
- Availability to work 9:00am - 5:30pm CT or ET, Monday through Friday
- Bilingual Spanish a plus, though not required
